Women's Hair Transplant — A Different Approach

Female hair loss presents differently from male pattern baldness — and requires a different surgical approach. Women rarely develop the defined bald zones seen in men. Instead, female pattern hair loss (FPHL) typically presents as diffuse thinning across the crown and midpart, with the frontal hairline usually preserved. Traction alopecia, hormonal loss and scarring alopecia are other common patterns in female patients.

At Hairmedico, Dr. Arslan Musbeh approaches female cases with specific assessment criteria: donor zone evaluation (which may also show diffuse thinning), the Ludwig scale of hair loss, hormonal history, and a frank assessment of whether surgical restoration is appropriate at the current stage.

Who Is a Candidate?

Female hair transplant candidates typically include women with: stable, defined hairline recession (frontal fibrosing pattern), traction alopecia from prolonged mechanical stress on the hairline, scar coverage from previous surgeries or injury, and established female pattern hair loss with a stable, adequate donor zone.

Women with active diffuse thinning across the entire scalp — including the donor zone — require more careful evaluation, as transplanting from a weakened donor zone risks graft failure. Dr. Arslan always assesses donor quality before recommending surgery.

The assessment priority for female patients: Is the donor zone stable and dense enough to harvest from? This question drives the entire surgical decision. Women with diffuse donor zone thinning may benefit more from medical management (minoxidil, PRP, hormonal evaluation) than surgery at this stage. See our treatment options guide →

Technique — FUE or DHI?

For female patients, DHI (Choi pen implantation) is frequently preferred because it allows grafts to be placed between existing hairs without shaving the recipient zone — maintaining coverage throughout recovery. FUE remains appropriate for specific cases, particularly hairline restoration where a clearly defined donor and recipient zone allows planned shaving. Dr. Arslan selects the appropriate technique based on each patient's specific anatomy and presentation.

Women's Results — Before and After

Every case below is a Hairmedico patient operated on by Dr. Arslan Musbeh in Istanbul, photographed before surgery and at follow-up. Female cases are almost always DHI implantation into existing hair, so what you see is density added between hairs that were kept — not a shaved scalp regrown from nothing. Graft numbers are shown as placed; tap a photo to enlarge it.

More cases, including men's and afro-textured results, are in the full before-and-after gallery.

What Has to Be Ruled Out First

Diffuse thinning in women has several causes that are not androgenetic, and most of them are treatable without surgery. Operating on loss that has a medical cause treats the symptom while the cause carries on — which is why the workup comes before the surgical plan, not after it.

Possible causeWhy it changes the answer
Thyroid disorderTreatable medically. Surgery on unstable loss can be spent for nothing.
Iron deficiencyA common and reversible cause of diffuse shedding.
Telogen effluviumShedding months after illness, surgery, crash dieting or major stress. Usually recovers on its own.
Postpartum sheddingFrequently resolves without intervention. Timing matters more than treatment.
Traction from stylingLoss from mechanical tension. The styling has to stop before anything else works.
Scarring alopeciaActive scarring conditions have to be quiet before grafts are placed.
What this means at consultation: you may be asked for blood work before a surgical date is discussed. A clinic that books a woman with diffuse shedding straight onto a surgery list, without asking why she is shedding, is not doing the assessment properly.

Recovery Without Cutting Your Hair

The concern almost every female patient raises first is whether her hair has to come off. With DHI implantation the recipient area is generally not shaved, and the donor area is handled the same way as in an unshaven procedure — trimmed only where extraction happens, covered by the hair above it.

Hairline and donor area marked directly through the patient's own hair — no shaving required to plan the procedure.
  • Days 1–10: careful washing, no tension on the scalp, hair worn loose.
  • Weeks 2–4: transplanted hairs shed. The hair around them masks most of it.
  • Tied styles: ponytails, buns and anything that pulls at the hairline wait until cleared — the tension that harms grafts is the same tension that causes traction loss.
  • Colouring: postponed until the scalp has fully settled. Ask at follow-up rather than guessing.
  • Heat and extensions: reintroduced last, and at lower tension than before.

If you are unsure which stage you are at, the Norwood and Ludwig scales are the quickest way to place it — or send photographs for an assessment of what your donor area can actually support.

Where Are You on the Ludwig Scale?

Female pattern loss is graded on the Ludwig scale rather than the Norwood scale used for men. The grade matters because it predicts two things at once: how much density a transplant can realistically add, and how much donor hair is likely to be available to add it with.

Ludwig gradeWhat it looks likeWhat surgery can do
I — mildWidening of the central parting; hairline intact; density still good at the sides and back.Usually medical treatment first. Surgery is reserved for a defined, stable thin zone — often the frontal parting.
II — moderateClear see-through thinning across the crown and mid-scalp; parting noticeably wider. The most common surgical candidate. DHI between the remaining hairs, 1,500–2,500 grafts in most cases.
III — advancedNear-complete thinning on top; the hairline may remain as a thin fringe.Depends entirely on the donor area. If the back and sides are also thin, surgery may not be advised — honesty here saves a wasted procedure.

Marking the parting and frontal zone on a Ludwig II patient — the plan is drawn where the density will be added, through the patient's own hair.

The scale is a starting point, not a verdict. Two women at the same grade can have very different donor areas, which is why photographs of the back and sides matter as much as photographs of the top. Read more in our guide to the Ludwig scale and female candidacy.

Designing a Female Hairline

A woman's hairline is not a man's hairline moved forward. It sits lower, it is rounder, and it has no temple recession to recreate — a design that copies the male template leaves a flat, masculine frame that no amount of density corrects. Dr. Arslan draws the line in consultation with the patient looking at it in the mirror, following the natural widow's peak and the soft, irregular edge that real female hairlines have.

Checked with the patient before anything is fixed — the design is approved by the person who will wear it.
  • High forehead (hairline lowering): women born with a high hairline, with no hair loss at all, can have the line brought forward by 1–2 cm. A stable donor area makes this one of the most predictable female procedures.
  • Thin or receded temples: the corners are filled with fine single-hair grafts so the edge stays soft. Over-dense temples look wig-like.
  • Angle and direction: grafts at the front edge are placed at a flat angle pointing forward and slightly down, matching how the surrounding hairs already grow — direction is what makes the result undetectable when the hair is pulled back.

See the dedicated page on hairline hair transplant for how the line is planned and placed.

Afro-Textured Hair in Women — Edges, Temples and Traction

Women with afro-textured hair are a distinct group of female patients, and the pattern they present with is different again. Androgenetic thinning on top is less often the complaint; far more common is loss at the edges and temples from years of braids, weaves, glued wigs or tight ponytails — traction alopecia — and, in some patients, central centrifugal cicatricial alopecia (CCCA), a scarring condition that starts at the crown.

Three things change in the plan:

  • Extraction: the follicle curves under the skin, so the punch and the angle are adapted to it. Done with a straight-hair technique, transection rates climb and grafts are wasted — see the afro hair transplant page for the technical detail.
  • Scarring history: keloid and hypertrophic scarring are more common in darker skin and are asked about before surgery — how old cuts, piercings and vaccination sites healed. A tendency changes punch size and donor spacing; it is rarely a reason not to operate.
  • The cause has to stop: edges restored under continued tension will go the same way as the original hair. Protective styles come back after healing, at a tension the scalp tolerates, with the timing set at follow-up.
CCCA is the exception. An active scarring alopecia has to be quiet — confirmed, where there is doubt, by a dermatologist and sometimes a biopsy — before grafts are placed into it. Operating into active CCCA loses the grafts. Where it has burnt out, transplanting the crown and parting is possible and often works well.

How Many Grafts, and What It Costs

Women's cases are usually smaller in graft count than men's, because the goal is density between existing hairs rather than coverage of a bare area. Typical ranges, from the cases above:

Case typeTypical graftsNote
Hairline lowering / high forehead1,200–1,800Single-hair grafts at the edge; donor usually excellent.
Traction edges and temples800–1,500Only once the tension has stopped.
Ludwig II parting and crown1,500–2,500DHI between existing hairs, no shave.
Scar coverage300–1,000Depends on scar blood supply; sometimes two small sessions.

Pricing at Hairmedico is per package, not per graft, so a 1,500-graft and a 2,500-graft case cost the same within the same tier. Sapphire FUE starts from €2,600 and DHI — the technique most women's cases use — from €3,200; the full package breakdown is here. A written quote follows the photo review, before you book anything.

Frequently asked questions

Answered by Dr. Arslan Musbeh.

Can women have hair transplants?

Yes. Women are excellent candidates for hair transplant when they have stable hair loss patterns, adequate donor density, and corrected underlying causes like thyroid or nutritional deficiencies.

Does women's hair transplant require shaving the head?

Not necessarily. Dr. Arslan Musbeh uses unshaven DHI technique for female patients when appropriate, allowing the procedure to remain discreet throughout recovery.

Which technique is used for women — FUE or DHI?

Most female cases are DHI, because grafts are implanted directly between existing hairs without shaving the recipient area. Sapphire FUE is chosen for defined hairline work or when a larger session is planned and shaving is acceptable. The choice is made after photo review, not before.

Can I have a hair transplant for a high forehead if I have no hair loss?

Yes. Hairline lowering brings the line forward by about 1–2 cm with single-hair grafts at the edge. Because the donor area is undamaged, it is one of the most predictable procedures in women — typically 1,200–1,800 grafts.

Do I need blood tests before a women's hair transplant?

Often, yes. Diffuse shedding can come from thyroid disorder, low iron, vitamin D deficiency or hormonal change, all of which are treatable without surgery. Dr. Arslan may ask for results before a surgical date is discussed, so that grafts are not spent on loss that has a medical cause.

Can women with afro-textured hair have a hair transplant?

Yes. The extraction punch and angle are adapted to the curved follicle, scarring history is checked beforehand, and the styling that caused traction loss has to have stopped. Active CCCA must be quiet before grafts are placed; burnt-out CCCA can be transplanted.

How many grafts does a woman usually need?

Typically 800–2,500. Edges and temples after traction loss need 800–1,500, a Ludwig II parting and crown 1,500–2,500, and hairline lowering 1,200–1,800. The exact number is set from photographs of the top and of the donor area.

What does a women's hair transplant cost in Turkey at Hairmedico?

Packages start from €2,600 for Sapphire FUE and €3,200 for DHI, priced per package rather than per graft, so a smaller female case does not pay less or more within its tier. The written quote follows the free photo review.

When can I tie my hair, colour it or wear extensions again?

Hair is worn loose for the first weeks. Loose ties are usually cleared at the first follow-up, colouring once the scalp has fully settled (ask at follow-up rather than guessing), and extensions or heat last of all, at lower tension than before.
