The brief

The surgery is the same. The design is not.

A feminising hairline and a masculine one are built from the same follicles toward opposite briefs. A typical male hairline sits higher, with defined temporal recessions and a straighter leading edge. A feminising hairline sits lower and rounder, with softer temporal points and a gentler transition zone. Getting that brief right matters more here than graft count does.

This is why the consultation carries more weight than in a routine case. The measurement that decides the result — where the hairline should sit for your face, not for a template — is agreed before surgery, in photographs and on the scalp, and it is your call to make.

Trans men more often come with the opposite problem: recession that began after starting testosterone, or facial hair that has not filled in. Both are ordinary restoration procedures. What differs is what the result is being designed toward.

Timing

What is assessed before a plan is made

Assessed	Why it changes the plan
Time on hormone therapy	Hormones change how hair behaves. A hairline designed while that is still shifting is designed against a moving target.
Whether the pattern is stable	As in any case, a hairline is drawn once the pattern has settled — not in the middle of a change.
Donor density	Sets what coverage is achievable, and whether scalp and facial hair work can both draw on the same supply.
Current hairline shape	Lowering a hairline and reshaping its corners are different amounts of work, and often different session sizes.
Full medical history	Hormone therapy is part of the surgical assessment like any other ongoing medication.

Dr. Arslan will say when waiting produces a better result. That is a judgement about timing, not a view about who should be treated.

Facial hair

Why beard work is not scalp work

Beard hair leaves the skin at a much shallower angle than scalp hair, and the direction changes across the face — upward along the jaw, downward through the moustache, forward at the chin. Grafts placed at a scalp angle stand away from the face instead of lying against it, and no amount of density fixes that afterwards.

Each zone is therefore planned separately rather than as one target count. The beard graft calculator gives a rough figure per zone, and beard transplant covers the procedure in full.

Because implantation angle carries this much weight, canal-opening is personally performed by Dr. Arslan Musbeh in the Gold and VIP tiers; extraction follows the package tier. Graft placement then follows the same structure as any other Hairmedico procedure: in the VIP tier he places every graft himself; in Silver and Gold, his trained team places them. The angle and direction plan for each zone is always his.

Patients combining beard density with scalp hairline work do not need two separate trips. A combined Head + Beard session covers both in one surgical day, planned as two distinct graft maps rather than one shared count.

Recovery

What recovery looks like, stage by stage

Stage	What to expect
Days 1–3	Swelling around the forehead and eyes settles. More noticeable after hairline or beard work than after crown-only sessions.
Days 4–10	Redness and small crusts around each graft resolve. Most patients are comfortable in public within this window.
Weeks 2–4	Transplanted hairs shed — a normal, temporary phase, not a sign the graft has failed.
Month 3	New growth begins — typically the beard zone first in FTM patients, then the scalp — and continues thickening for months afterward.
Month 12	Full, permanent result. Ongoing hormone therapy does not change this: transplanted follicles come from the donor zone at the back of the scalp, which is naturally resistant to DHT.

Beard grafts sit on visible, hard-to-cover skin, so most FTM patients plan the first 10–14 days around limited social exposure. Do I need to be post-op to have a hair transplant?+

No. Hair transplant can be performed at any appropriate point in transition. For MTF hairline work, 12+ months of HRT stability is recommended.

Will HRT affect my transplant result?+

HRT oestrogen in MTF patients reduces androgenetic recession. Timing the transplant after HRT stabilisation (12 months) generally gives the most predictable result.

For FTM patients, when is beard transplant appropriate?+

After testosterone levels have stabilised and beard growth potential is clear — usually 2–3 years on T.

Is everything confidential?+

Absolutely. Hairmedico is a private single-patient clinic. Complete confidentiality is guaranteed.

Does hormone therapy after surgery affect the transplanted hair?+

No. Transplanted follicles come from the donor zone at the back of the scalp, which is naturally resistant to DHT — the same biological basis used in every Hairmedico procedure. Continuing, adjusting or later changing hormone therapy does not affect grafts that have already taken.
