Keloid Risk in Afro Hair Transplant —
How to Prevent It and Who Is at Risk

Keloid scarring risk in Afro hair transplant: who is at risk, how FUE minimises it vs FUT, and how Dr.

What is a keloid scar?

A keloid is an overgrowth of scar tissue that extends beyond the boundaries of an original wound. Unlike a normal scar that remains flat and confined to the injury site, a keloid raises above the skin surface, feels firm, and may continue growing over months or years. In hair transplant surgery, keloids are the most feared long-term complication for patients of African descent.

People of African descent have a significantly higher risk of keloid formation than European or East Asian patients — estimated at 6–16% compared to 1–2% in the general population. This is not universal among Black patients, but it is common enough that it must be assessed before any surgical procedure involving skin incisions.

Dr. Arslan Musbeh — Afro & Textured Hair Expert
Dr. Arslan Musbeh has treated patients with Afro-textured, Type 3C–4C and tightly coiled hair from West Africa, Central Africa, the UK, France and the Caribbean. His adapted FUE technique accounts for the C-shaped follicle curvature unique to Afro hair — minimising transection rates that conventional punches cannot avoid. As a FUE Europe Full Member with 17+ years and 4,500+ personal cases, Dr. Arslan is one of the few Istanbul surgeons with documented expertise specifically in Afro-textured hair restoration.

FUE vs FUT — why FUE is strongly preferred for Afro patients

FUT (strip surgery) involves removing a linear strip of scalp from the donor zone and suturing the wound closed. This creates a linear scar across the back of the head — a large incision that, in a keloid-prone patient, can produce a raised, thickened scar that is both visible and uncomfortable.

FUE creates only tiny circular extraction points of 0.9–1.0mm diameter at Hairmedico. These heal as small dot scars — a fraction of the surface area of an FUT incision. In keloid-prone patients, smaller incisions mean dramatically lower risk of keloid formation.

For this reason, FUE is the strongly preferred technique for patients of African descent at Hairmedico — regardless of the graft count required.

Pre-operative keloid risk assessment

Before any Afro hair transplant, Dr. Arslan performs a keloid risk assessment. This involves reviewing the patient's personal and family history of keloid formation, examining any existing scars on the body for keloid characteristics, and assessing skin type and tone.

Patients with a confirmed history of keloid formation require additional consideration: pre-operative intralesional corticosteroid injection protocols, modified post-operative care including silicone sheeting, and in some cases a test session (a small number of grafts placed and observed for 3 months before proceeding with a full procedure).

Post-operative keloid prevention protocol

At Hairmedico, all Afro hair transplant patients receive a modified post-operative protocol regardless of confirmed keloid history:

  • Gentle handling of donor sites — no aggressive scrubbing or picking of extraction scabs
  • Silicone-based healing gel applied to donor zone from day 10
  • Sun protection for donor and recipient zones for 3 months
  • Monthly WhatsApp photograph review — any abnormal scar formation is identified early and managed with intralesional corticosteroid before it progresses

Questions

Common questions

How much does a hair transplant cost in Turkey?

Hairmedico all-inclusive: price on request for 2,000–3,500 grafts including surgery, 5-star hotel, transfers, PRP and 12-month follow-up.

Is hair transplant permanent?

Yes. Transplanted follicles are DHT-resistant and grow permanently for the patient's lifetime.

What is the recovery time?

Desk work: 5–7 days. Scabs shed by Day 10–14. New growth from months 3–5. Final result months 12–14.

Who performs surgery at Hairmedico?

Dr. Arslan Musbeh personally performs every extraction and canal-opening. In VIP he also places every graft himself; in Silver and Gold, his trained team places the grafts under his direct supervision. One patient per day.

Is Turkey safe for hair transplant?

Yes, with an accredited surgeon-led clinic. Verify FUE Europe membership, Ministry of Health registration, and confirm the surgeon personally operates.
