Why medical treatment matters
Transplanted hair is DHT-resistant and permanent. Native hair surrounding the transplant is not. Without medical treatment, native hair continues miniaturising, leaving transplanted islands increasingly isolated. This is the most common cause of unnatural-looking results at 5-10 years. Finasteride - how it works
1mg daily. Inhibits type II 5-alpha-reductase, reducing DHT conversion by 70%. Stabilises androgenetic loss in 85% of users. Must be continued indefinitely - stopping reverses benefit within 12 months. Side effects: sexual dysfunction in 1-2% (typically reversible). Contraindicated in women of childbearing potential.
Clascoterone - the topical alternative
Topical androgen receptor blocker at follicle level. No systemic DHT suppression. Indicated for Finasteride-intolerant patients and potentially women. Does not create follicles - slows miniaturisation of existing ones. Can be combined with Finasteride for additive effect. Hair Transplant Turkey

Why is medical treatment needed after a hair transplant if the grafts are permanent?
Transplanted follicles are DHT-resistant, but the native hair around them is not. Without Finasteride or Clascoterone, that surrounding hair keeps miniaturising, which is the most common cause of unnatural-looking results at 5-10 years.
What happens if a patient stops taking Finasteride after surgery?
The protective effect reverses within about 12 months of stopping. Finasteride only works while it is being taken, so it must be continued indefinitely to keep protecting the native hair surrounding the transplant.
Can Finasteride and Clascoterone be used together before or after a transplant?
Yes. Clascoterone works locally at the follicle without affecting systemic DHT, so it can be combined with oral Finasteride for an additive effect, or used alone in patients who are Finasteride-intolerant.
