How common is hair loss in Black women?

Hair loss in Black women is significantly underreported in clinical statistics, but prevalence studies suggest it is extremely common. Traction alopecia alone may affect 30–40% of Black women at some point in their lives. CCCA affects an estimated 5–17% of Black women in the US and UK. Female androgenetic alopecia adds further to this prevalence. The combined burden of hair loss conditions in Black women is one of the most underserved areas of dermatology globally.

1. Traction alopecia — the most common cause

Caused by repeated or sustained mechanical tension on the follicle from tight hairstyles. Braids, cornrows, weaves, extensions, tight ponytails and relaxer-tension combinations are the primary offenders. Begins at the edges, temples and nape — the finest, most vulnerable hair. Reversible early; permanent after prolonged follicle destruction.

2. CCCA — scarring alopecia specific to Black women

Central Centrifugal Cicatricial Alopecia begins at the crown and spreads outward with permanent scarring. Associated with hairstyling practices, possible genetic predisposition and certain hair care products. Often underdiagnosed because it progresses slowly and may be initially mistaken for androgenetic alopecia.

3. Female androgenetic alopecia

DHT-driven follicle miniaturisation following the Ludwig pattern — diffuse thinning across the central scalp with preserved frontal hairline. Treated medically with Minoxidil and spironolactone. Hair transplant is appropriate once stable pattern is confirmed and donor area is assessed as adequate.

4. Other contributing factors

Chemical damage: Relaxers, bleaching and repeated colour processing weaken the hair shaft and, in extreme cases, damage follicles. Primary recovery through cessation and scalp healing.

Postpartum hair loss: Telogen effluvium following pregnancy is common and typically self-resolving within 6–12 months. Does not typically require surgery.

Nutritional deficiency: Iron, ferritin, vitamin D and zinc deficiencies all contribute to diffuse hair thinning. Blood test required before any surgical consultation.

When is hair transplant appropriate?

Surgery is appropriate when: the cause of loss is confirmed as permanent (follicles no longer viable), the donor area is healthy and adequate, the underlying condition is stable, and medical treatments have been exhausted or are insufficient. For traction alopecia, surgery can be highly effective once the causative style is discontinued. For CCCA, surgery requires confirmed disease remission of 12–24 months.
