What makes Afro hair transplant different?

Afro-textured hair presents a unique surgical challenge that distinguishes it from straight or wavy hair transplantation. The follicle of Afro hair is not straight — it follows a curved, C-shaped or S-shaped path beneath the scalp surface. This means that a standard FUE punch inserted perpendicularly risks cutting through the follicle before fully extracting it — a complication known as transection.

In standard FUE on straight hair, transection rates are typically 2–5%. With Afro hair, using conventional punches without technique adaptation, transection rates of 30–80% have been documented in clinical literature. This is not an acceptable outcome — each transected follicle is a permanently lost graft.

Hair loss in Afro men — the main causes

Androgenetic alopecia (male pattern baldness)

DHT-driven follicle miniaturisation follows the same Norwood pattern in men of African descent as in men of other ethnicities — receding temples, crown thinning, and eventual vertex baldness. The progression rate and pattern vary by individual genetics, not ethnicity.

Traction alopecia

Tight hairstyles — dreadlocks, cornrows, braids, tight fades with elastic bands — exert sustained tension on the hairline follicles. Repeated over years, this tension permanently damages the follicle. Traction alopecia in men typically manifests as hairline recession at the temples and frontal edges — the areas most affected by styling tension.

Pseudofolliculitis barbae

Ingrown hairs caused by the tight curl of Afro hair growing back into the skin. Chronic inflammation from this condition can damage follicles in the beard and nape zone if untreated over years.

  • Punch size: 0.9–1.0mm (vs 0.7–0.8mm for straight hair) to accommodate the larger follicle diameter
  • Entry angle: The punch follows the exit angle of the hair at the skin surface, then adjusts for the expected curl beneath — reducing the chance of cutting across the curved follicle
  • Shallower scoring: Initial scoring at 1–2mm depth, then follicle mobilisation with lateral oscillation rather than a single downward punch
  • Graft inspection: Every extracted graft is inspected under magnification before implantation — transected grafts are identified and set aside
  • Session sizing: Maximum 3,500–4,000 grafts per session for Afro hair — the additional care required per graft limits safe session size relative to straight hair

Results — what to expect

One advantage of Afro hair is its natural volume. A single coiled hair strand covers more surface area than a straight hair strand of equivalent density — meaning that 2,000 grafts of Afro hair typically produces a visually denser result than 2,000 grafts of fine straight hair.

The growth timeline is standard: shock loss at weeks 2–6, dormant phase to month 3, new growth from month 4, 70–80% density by month 8, final result at 12–14 months. Transplanted Afro hair maintains its original curl pattern — the grafts grow with the same texture as the donor hair.
