What is Type 4 / 4C hair and why does it matter in transplant surgery?

Hair typing systems (such as the Andre Walker system) classify hair into Types 1–4 based on curl pattern. Type 4 hair — subdivided into 4A (soft coils), 4B (Z-shaped pattern), and 4C (tightest coils, minimal definition without product) — has the tightest curl pattern of any hair classification.

In hair transplant surgery, Type 4 / 4C hair is the most technically demanding to work with because its follicle curvature beneath the scalp is the most extreme. The C-shaped follicle path is more pronounced, the diameter larger, and the risk of transection during extraction highest.

A surgeon unfamiliar with Type 4 hair structure may achieve adequate results with Type 3C (loose curl) hair using mild technique adaptations. Type 4C requires a fundamentally different approach — it is not a matter of degree but of category.

Specific challenges with 4C hair transplantation

  • Maximum follicle curvature: 4C follicles may bend 60–90° beneath the skin — making standard punch extraction almost certain to transect
  • Follicle fragility: The tight coil structure under the skin creates tension in the follicle wall — making the follicle more sensitive to mechanical and dehydration stress during storage
  • Shrinkage: 4C hair has the highest shrinkage rate of any hair type — a strand may appear 3–4cm when stretched but coil to under 1cm when relaxed. This must be accounted for in density planning
  • Density per cm²: Natural 4C hair density is typically 50–70 FU/cm² — lower than straight hair (80–100 FU/cm²). This affects how aggressively the donor can be harvested

What 4C hair transplant results look like

Successfully executed 4C hair transplant produces some of the most visually striking results in hair restoration — precisely because the tight coil provides maximum volume per hair strand. A hairline restored with 800 grafts of 4C hair appears dramatically different from a receding or thinning hairline, because each transplanted coil expands to multiple times its straight-hair-equivalent volume.

The transplanted hair grows with the same 4C curl pattern as the donor. There is no change in texture. The restored area integrates seamlessly with surrounding natural 4C hair.
