The most common question from loc wearers

"I have locs — can I get a hair transplant?" and "Will I have to cut my locs for surgery?" are two of the most frequently asked questions at Hairmedico consultations for Afro-textured hair. The answers are nuanced but generally positive for patients whose locs have not caused permanent damage.

The donor area — what happens to your locs during surgery

FUE surgery requires shaving the donor area (back and sides of the scalp) to allow precise extraction of individual follicular units. For patients with locs or dreadlocks, this means the donor zone hair must be cut short — typically to 1–2mm — before extraction begins.

At Hairmedico, Dr. Arslan works with the patient before surgery to plan the shaving area precisely, minimising the extent of shaved donor area to what is clinically necessary. For smaller sessions (1,500 grafts or fewer), a partial shave of the donor zone may be sufficient — leaving the majority of the locs intact.

The donor zone hair grows back normally within 3–6 months. Patients can then re-establish locs in the donor area. The transplanted hair grows back as the patient's natural texture — which will eventually loc if the patient chooses.

If your locs have caused hairline recession

Dreadlocks and locs cause traction alopecia when they are heavy, very long, or styled with tension at the roots. If your locs have caused recession at the temples, frontal hairline or nape, the first step is to assess whether that loss is permanent.

Dr. Arslan performs trichoscopy at every consultation to check whether follicles remain viable in recession zones. Early traction from locs is often reversible once the weight and tension is reduced. Permanent loss can be restored with FUE.

After surgery — when can you loc again?

The transplanted grafts require 3–6 months of gentle handling before any styling tension is applied. Locking the transplanted hair — through any technique — must be delayed until the grafts have fully integrated and the follicles are firmly established. Dr. Arslan provides specific post-operative guidance for loc-wearing patients as part of the Hairmedico 12-month follow-up programme.
