Female hair loss — more common than you think

Approximately 40% of women experience noticeable hair loss by age 50. Despite this prevalence, female hair loss receives significantly less clinical attention than male hair loss and is less openly discussed. Women experiencing hair loss often feel isolated — the condition is both common and frequently misunderstood, including by some clinicians.

Hair transplant surgery for women has become increasingly mainstream. Women now account for 15–20% of hair restoration patients globally, and the clinical outcomes for appropriately selected female candidates are excellent.

Types of female hair loss treated by transplant

Why female cases are technically different

Female hair transplant cases require different clinical planning from male cases for three key reasons:

1. Diffuse donor zone: Unlike men where the donor zone at the back and sides is reliably DHT-resistant, women with FPHL may have follicle miniaturisation across the entire scalp — including the donor area. This makes donor assessment by trichoscopy essential before confirming candidacy.

2. No shaving required: Most women cannot shave their heads for social or professional reasons. At Hairmedico, unshaven FUE is standard for female patients — only small sections of the donor area are shaved, concealed by surrounding hair. The recipient area is never shaved.

3. Hairline aesthetics: Female hairlines follow different principles than male hairlines — different height, different curvature, different temporal shape. Surgical design for women requires specific aesthetic expertise.

Am I a candidate?

Surgery is most appropriate for women when:

Surgery is not appropriate when loss is active and progressive, when the donor zone shows diffuse miniaturisation, or when an autoimmune condition is in active phase.

The consultation process for women

At Hairmedico, female consultations include: detailed history (onset, pattern, hormonal events, medications), trichoscopic assessment of both recipient and donor zones, review of blood panel results if available (ferritin, thyroid, androgens, vitamin D), standardised photographs and discussion of medical alternatives alongside surgery.

For women where surgery is not the immediate recommendation, Dr. Arslan will propose a medical management plan — minoxidil, spironolactone if appropriate, or PRP — with a clear threshold for when surgery would become indicated.

What results women can expect

For traction alopecia and hairline-lowering cases — where there is a clearly demarcated area to restore — results are typically excellent and highly predictable. The transplanted follicles grow hair with the same texture and character as native hair. Timeline is identical to male cases: visible growth from month 3–4, significant density at month 6, full result at 12–14 months.

For diffuse FPHL cases, results depend on donor zone integrity. Women with strong, unaffected donor zones achieve significant density improvements. Conservative planning is essential to avoid depleting a potentially limited resource.
