The real growth timeline — why month 3 feels like failure

Month 3 post-transplant is statistically the point of highest patient anxiety — and the point of least visible progress. Understanding why requires understanding the hair growth cycle.

After surgery: transplanted hairs shed at weeks 2–6 (shock loss). The follicles then enter a resting (telogen) phase that typically lasts 8–14 weeks. During this dormant period, there is essentially no visible growth. This is entirely normal — the follicles are alive and recovering, but have not yet entered the active growth (anagen) phase.

Nouveau growth typically begins at months 3–5 as follicles enter anagen. Growth is initially slow and the new hairs are fine. By month 6, 40–60% of the final result is visible. By month 9–10, 80–90%. Final result at months 12–14.

Actual reasons transplanted hair may not be growing well

1. You are simply at the dormant phase (months 1–3)

The most common reason. If it has been less than 4 months: wait. Growth is coming.

2. Poor graft survival from the surgery

If graft survival was below 80–85%, the result may be noticeably less dense than expected. Causes include: high transection during extraction, grafts stored incorrectly, excessive out-of-body time, desiccation during implantation. This is preventable with high-quality surgical technique — it should not occur at an experienced, surgeon-led clinic.

3. Post-operative damage to grafts

Grafts dislodged in the first 10 days — from touching, vigorous washing or physical trauma — are permanently lost. If the patient did not follow post-operative care instructions carefully, this may reduce effective graft count.

4. Medical factors

Nutritional deficiency (iron, ferritin, vitamin D), thyroid dysfunction, or other systemic conditions can slow the recovery of transplanted follicles. A blood panel is worth checking if growth is significantly below expected at 6 months.

What to do — a step-by-step guide

  1. Avant month 5: Do nothing except follow post-operative instructions. Growth is likely coming.
  2. Month 5–6: Take comparison photographs (same lighting, same angle) from consultation day. Even small improvement is usually visible.
  3. Month 7–8: If no visible growth at all, contact your clinic for a trichoscopy assessment to evaluate follicle viability.
  4. Month 12+: Final result assessment. If density is significantly below expectation, explore whether a supplementary session is appropriate.

Is it normal for transplanted hair not to grow at 3 months?+

Yes. Month 3 is the slowest phase — the shock loss has completed and new growth has barely begun. Most patients see little visible change between months 1 and 4. Significant growth typically begins at months 4–5.

Why is my transplanted hair still not growing at 6 months?+

At 6 months, 40–60% of the final result should be visible. If you see less, check: are any hairs growing at all (even fine ones)? If yes, patience is required — growth rate varies significantly by individual. If no growth at all by month 7–8, a clinical assessment is warranted.

When should I be worried about transplanted hair not growing?+
