Why the crown is the most challenging area

The crown — or vertex — is the circular area at the top of the head. From a surgical perspective, it presents three unique challenges that the hairline does not.

1. The vortex: Crown hair grows in a circular, whorl-like pattern radiating outward from a central point. Matching this natural growth pattern during implantation requires exceptional precision and experience.

2. Progressive loss: The crown is typically the last area to reach its final stage of loss — and it can continue expanding for decades. A 35-year-old patient who receives crown grafts today may find the transplanted area is surrounded by further loss at 45 or 55, requiring additional procedures or leaving an unnatural isolated island of hair.

3. Optical illusion: The crown is a concave area — hair does not sit flat but falls away from the centre in all directions. This means 40 FU/cm² at the crown looks significantly less dense than the same number at a flat frontal region. Managing patient expectations about crown density is one of the more difficult conversations in hair restoration.

How Dr. Arslan plans crown cases

The correct approach to crown treatment depends heavily on age, current Norwood stage, family history and donor capacity.

For patients under 35:

Crown treatment is generally not advised as a standalone procedure for younger patients with active progressive loss. The risk of creating an isolated patch of transplanted hair surrounded by future loss is significant. For younger patients, frontal reconstruction is typically prioritised — the frontal hairline has the greatest impact on appearance and frames the face.

For patients 40 and older with stable crown loss:

Crown treatment can be an excellent option when frontal reconstruction is already complete or not required, donor capacity permits, and progression appears stable. Staged treatment — frontal first, crown second — is often the most prudent approach.

Combined cases:

In Norwood IV–VI patients with sufficient donor density, a single session covering both the frontal zone and crown is achievable. This requires a careful distribution plan — prioritising density at the frontal hairline while placing grafts in the crown at a lower, sustainable density.

What density can you realistically achieve at the crown?

Managing expectations about crown density is essential. Because of the vortex growth pattern and the optical effect of a convex surface, crown results always look less dense than frontal results using the same graft numbers.
