Why Afro hair needs fewer grafts for equivalent coverage

One of the most important — and least understood — aspects of Afro hair transplantation is that coiled hair provides significantly more visual coverage per graft than straight hair. The reason is geometric: a tightly coiled hair strand occupies a larger surface area on the scalp than a straight strand of equivalent length, because the coil creates volume in multiple dimensions rather than lying flat.

Studies and clinical observations suggest that 2,000 grafts of Type 4 Afro hair can produce coverage equivalent to 3,000–3,500 grafts of fine straight hair. This is a meaningful difference that affects both cost and donor preservation planning.

Dr. Arslan Musbeh — Afro & Textured Hair Expert
Dr. Arslan Musbeh has treated patients with Afro-textured, Type 3C–4C and tightly coiled hair from West Africa, Central Africa, the UK, France and the Caribbean. His adapted FUE technique accounts for the C-shaped follicle curvature unique to Afro hair — minimising transection rates that conventional punches cannot avoid. As a FUE Europe Full Member with 17+ years and 3,000+ personal cases, Dr. Arslan is one of the few Istanbul surgeons with documented expertise specifically in Afro-textured hair restoration.

Graft estimates for Afro hair by hair loss pattern

Hair loss patternTypical graft estimate (Afro)Equivalent straight hair estimate
Hairline recession only (traction / early AGA)600–1,200900–1,800
Norwood II–III — temple + frontal recession1,200–2,0001,800–2,800
Norwood III–IV — frontal + mild crown1,800–2,8002,500–3,500
Norwood IV–V — significant bald area2,500–3,5003,500–4,500
Maximum single session (Afro)3,500–4,0004,500–5,000

These are estimates. Actual graft requirements are determined by individual scalp size, target density, hair calibre, donor density and the patient's long-term hair loss trajectory.

Donor capacity in Afro hair

Afro hair donor zones contain slightly fewer follicular units per cm² than straight hair donor zones (approximately 60–80 FU/cm² vs 80–100 FU/cm²). This means the total lifetime donor supply is somewhat smaller — a consideration in planning multi-session approaches for advanced Norwood stages.

At Hairmedico, every Afro hair assessment includes a trichoscopy scan of the donor zone to measure actual density in grafts per cm². This provides a precise total donor capacity figure rather than a population average estimate — allowing accurate session planning and lifetime donor management.
