What is a hair graft?

A hair graft — also called a follicular unit — is a naturally occurring group of 1 to 4 hair follicles that grow together from the same follicular structure. When surgeons talk about graft counts, they are counting these groups, not individual hairs.

The average graft contains 2–2.5 hairs. This means a procedure of 2,000 grafts typically yields 4,000–5,000 individual hairs — the distinction matters when comparing clinic quotes.

Always ask clinics for graft count, not hair count. Some clinics quote procedures in individual hairs to make numbers appear larger.

Graft estimates by Norwood stage

The primary factor determining how many grafts you need is your Norwood stage — the extent of hair loss. These are clinical estimates; individual anatomy, hair characteristics and coverage goals will affect the final number.

These figures assume standard scalp coverage goals. A more conservative hairline design requires fewer grafts. A higher density target requires more.

Density — how grafts per cm² affects your result

The number of grafts is only part of the equation. Density — how many grafts are placed per square centimetre — determines how thick and natural the result looks.

A 40–60 FU/cm² density produces a natural result in most patients — the human eye cannot detect the difference from native density at this level. Attempting to exceed the donor zone capacity is the most common cause of overharvested, depleted donor areas in revision cases.

Donor capacity — the limiting factor

The maximum number of grafts available from your scalp is determined by your donor zone density and the surface area of the safe donor zone. This varies significantly between individuals.

Average donor zone: approximately 6,000–8,000 extractable grafts over a lifetime. High-density donors may yield 9,000–10,000. Low-density donors may be limited to 4,000–5,000.

This is why donor assessment — not just loss assessment — is critical in surgical planning. A Norwood VI patient with a high-density donor can achieve excellent coverage in one session. The same patient with a limited donor zone may require a more conservative plan across two sessions.

Additional donor sources: in some cases, body hair from the beard, chest or legs can supplement scalp grafts. Body hair grafts are coarser and shorter, typically suited to adding density to the mid-scalp rather than the hairline.

An accurate graft estimate cannot be given from a standard photograph alone. It requires:

  1. High-quality photographs of all affected areas (top, front, back, both sides)
  2. Assessment of donor zone density and distribution
  3. Discussion of your coverage goals and long-term hair loss trajectory
  4. Review of your family history to model future loss
